JD and Turk’s college buddy Spence has come for a visit. In their college days, Spence really stirred things up, getting them into trouble and things haven’t changed. While being introduced to Dr. Cox and Jordan, he congratulates them on their baby and then proceeds to blurt out the secret that JD had been hiding - that Dr. Cox is the father of Jordan’s baby. A stunned Dr. Cox tells Jordan that he’ll be there for the baby, but he doesn’t think they can be together. Jordan tries to justify her lie by telling him that she didn’t want to manipulate him into being with her just because they had a baby together. Jordan decides that she is going to harass Dr. Cox until he gives in and takes her back.
Spence gets JD and Turk into trouble again when they end up partying and drinking in a hot tub with their neighbors. The problem is that they are technically on call. Sure enough both JD and Turk’s pagers go off because there is a multiple casualty emergency at the hospital. They get to the hospital late where Dr. Cox starts yelling at them because he smells beer on their breath.
JD and Turk confront Spence about getting them into trouble. Spence says that they are really angry about their own medical careers, which up to this point, haven’t been very rewarding, personally or financially. But after reflecting on their lives, they conclude that their purpose in life, and the reason they went into the medical field, is to help people.
Elliot accidentally sticks Dr. Kelso with a needle, which causes him to go on a harsh tirade of Elliot’s inabilities that upsets her more than ever before. She turns to Dr. Cox for help, but he’s too busy with his own problems. She turns to Ted, the lawyer, but he can’t help her either.
Dr. Cox finally gives in to Jordan’s constant badgering and decides to be a family. Dr. Cox tells JD that he is afraid of being a father, but JD tells him that he’ll be fine because he is always there for the residents.
JD and Dr. Cox walk into Admissions where they find Dr. Kelso criticising Elliot extremely harshly in front of everyone. A frustrated and fed up Dr. Cox, strolls over to Dr. Kelso and punches him in the nose, leaving everyone stunned. Dr. Cox then strolls out of the hospital to be with his newfound family.
