JD and his new girlfriend Kim are deciding if they should keep their baby, whilst Carla goes into labour. JD and Turk make a big deal about it which includes a marching band, but their efforts seem to cause more chaos to the expectant mother. Turk's efforts go from bad to worse when the footage of the stages of the birth, being filmed so the family in Chicago can watch, accidentally pops up on all the televisions around the hospital.
JD and Kim make a list of pros and cons about keeping the baby or not. There are more cons than pros but JD’s eyelash falls out, giving him a wish, making the decision impartial. The task becomes worse, when the coin they toss lands on its edge, this leads JD to soul-search. Also Laverne and Jordan stick their oar in about what JD and Kim should do.
As well as JD feeling the pressure Jordan is mad at Dr Cox for talking to their son as if he is a drinking buddy. This leads to the toddler telling a lady she is a ‘fatty’ and telling Dr Kelso that his skin is wrinkly.
Meanwhile Elliot is taking over caring for Carla and leaving Turk to feel useless and to deal with the marching band. Turk eventually confronts Elliot about taking over, so Elliot gives him responsibility for getting the ice chips. This simple task leads to Turk getting his hand stuck in the ice machine. He is forced to move the machine to door of Carla’s room when he receives the news she has to the delivers their child through C-section. He is able to join his wife when Kelso presses the release button on the machine and Carla gives birth to Isabella Turk.
