After watching Al Gore's environmental film An Inconvenient Truth, the Janitor vows to save the world by becoming the Environmental Officer at Sacred Heart, but the staff soon find out that his methods of motivation are a bit peculiar, including a carpool lane in the car park and issuing out punishments for offendors, even though he has no power to do so.
Meanwhile, JD has to face his own inconvenient truth when his brother, Dan returns to town and tells him that it is time to 'grow-up'. Elsewhere, Elliot and Dr. Cox debate over the hypocrisy of their practice, after Elliot has criticised Cox for screaming at someone and then telling a patient not to stress over little things.
