This year sees JD finishing his residency and becoming a full-blown colleague of his idol and tormentor Dr. Cox and co-chief resident, along with Elliot. This change in rank doesn’t really affect the way he's treated, and JD is still bullied and insulted by his superior. Turk and Carla are back from their honeymoon, but the marriage soon hits problems when Carla decides her new husband should change to improve. And there’s more trouble when JD and his best friend’s wife share a mistaken drunken kiss.
Dr. Cox and Jordan get a surprise when they learn that their supposed divorce was not final, but this seemingly good news causes trouble. Elliot is still unhappy with JD for the woes with their relationship and exacts some revenge by dating his brother. But JD has a distraction of his own in the form of a new love interest, Dr. Molly Clock, an attractive psychiatrist, who becomes Elliot’s mentor at Sacred Heart.
This year also sees the purchase of some scooters, the Sugar Hill Gang, a light bulb in an odd place, fake heart attacks, quarantine, tacos, motorized wheelchairs and Colin Farrell.
