The fifth year for the young medics at Sacred Heart hospital begins with JD living in a hotel. Elliot leaves the hospital to a new fellowship elsewhere. Turk and Carla are trying to start a family, despite Turk still having doubts. And a new influx on interns at Sacred Heart brings joy and the inevitable complications.
JD discovers he can’t help but treat the new interns badly, even though he tries desperately not to do so. To prove his manhood he takes part in a triathlon, and gets set up on a blind date with a patient’s niece. A booty call backfires and he has to deal with the death of one of his favourite patient’s. He’s also let down considerably by his idol Dr. Cox and spends some time in the hospital’s water tower.
Dr. Cox struggles to cope with the death of several patients and is forced to take a leave of absence and returns to work with a lack of confidence. He’s also visited by his born again Christian sister Paige, who he desperately tries to avoid. He is forced to question his abilities as a father and soon as to face the possibility of another bun in the oven.
Elliot tries to keep her cool as she starts work at the County Hospital. Her residency there ends and after a spell at a free clinic, she’s soon back at Sacred Heart. Her own particular ‘booty call’ with one of the new interns soon develops into something more serious and she befriends JD’s new love interest, which can only lead to trouble.
Carla and Turk are considering having a baby, which Turk gets nervous and later Carla has doubts of her own. Turk has to deal with a patient who wishes to have hypnosis rather than anaesthesia and gets into trouble when it’s discovered he visited a patient’s home unnecessarily. As they continually fail to conceive, Carla questions Turk’s fertility and also has doubts about her own age and abilities. But when they are finally successful, they can’t think of a name for the baby.
